Exemplar Care partners with CISS to provide on-site healthcare at the shelter, removing the transportation and cost barriers that often prevent individuals from receiving medical services. The clinic provides quality medical care and assists with coordination of mental and behavioral health service needs.
The care and case management team works to identify the integrated health care needs of patients, discover any social determinants of health (SDOH) barriers, and facilitate follow-ups when needed. In addition, the team assists clients with applications for presumptive Medicaid/Medicare and advocates to increase access to resources and services that improve overall health.
Exemplar Care also partners with CISS to meet with and identify the potential healthcare needs of those who are homeless but not utilizing the shelter.
